Certified Registered Nurse Pr! actitioner in Pennsylvania.
Certificate of Prescriptive Authority in Pennsylvania.
National Certification as a Nurse Practitioner in area of education/subspecialty (Pediatric Acute Care/Critical Care).

Master’s degree from an NLNAC and/or CCNE accredited graduate school of nursing, in a Pediatric Acute Care/Critical Care program.

2 years Nurse Praxtitioner experience required.

Pediatric cardiology experience required.

All CHOP employees who work in a patient building or who provide patient care are required to receive an annual influenza vaccine unless they are granted a medical or religious exemption.

Job Description:
Cardiac Step Down (Cardiac Care Unit) Nurse Practitioner. Weekend Position. Requires rotation to day/evening/night/weekend/holidays.

In collaboration with an attending physician, the Acute Care Nurse Practitioner is responsible for the direct care and follow-up of a population of patie! nts that matches his/her scope of practice and skill set. The ! components of the role include clinical practice, consultation, professional development, education, and research.

Health care services provided by Nurse practitioners in the Cardiac Center include but are not limited to the following:

Assessment and Diagnosis

  • Obtain comprehensive health history
  • Perform physical examination
  • Order and interpret screening and laboratory tests
  • Order and interpret common diagnostic procedures
  • Assess and diagnose congenital & acquired cardiac disease and health related concerns and issues that fall within the expertise and knowledge of the NP
  • Formulate a family centered health care plan
Intervention
  • Treat cardiac, cardiothoracic surgical, pulmonary hypertension, heart failure, and heart and lung transplant, patients according to the guidelines of the Division of Cardiology
  • Prescribe medications related to management & health concerns of t! he above patients according to the guidelines of the formulary of The Children’s Hospital of Philadelphia
  • Perform the following procedures:
o Intraosseous infusion and electrical cardioversion or defibrillation per PALS protocol
  • Provide patient/family education regarding purpose, regimens and side effects and possible interactions of medications and/or treatments.
  • Provide any other appropriate interventions within the parameters, expertise and knowledge of the NP
  • Provide appropriate follow up care
  • Consult with physicians and other health care providers when necessary
  • Coordinate patient care and make appropriate referrals
  • Identify community resources and coordinate referrals for problems beyond the scope of practice
  • Advocate for patients and their families
Evaluation
  • Monitor and evaluate effectiveness of prescribed treatment plans
  • Monitor child and family! response to treatments
  • Modify interventions based on effectiv! eness, evidence-based practice and individual child and family needs
  • Monitor effectiveness of practice
Patient Populations
  • All patients new to the Cardiac Center will be seen with an Attending Physician to establish a diagnosis and develop a treatment plan
  • In subsequent visits, in collaboration of with the physicians of The Cardiac Center, NP’s will see patients that include but are not following:
o Established patients s/p cardiac surgery
o Established patients s/p cardiac catheterization
o Established patients with congenital heart disease
o Established patients with arrhythmias
o Established patients with acquired heart disease
o Established patients with heart failure
o Established patients with pulmonary hypertension
o Established patients s/p heart and lung transplants
o Others as determined with the attending physician
  • NPs will consult the attending physician as needed! , specifically if there is a change in the patient’s condition warranting a change in the established plan of care
  • NPs may provide care to established patients over the age of 21years of age. The NP will consult with the attending physician if this patients care is outside of his/her scope
