UPMC Hamot is hiring a Full-Time Clin II Home Care Nurse to help support the HAMOT-HH Nursing Department for its UPMC Hamot location.
To utilize th! e nursing process to diagnose and treat human response to illness in the home. To execute nursing practice through direct implementation of nursing care or through assisting patient care to other qualified personnel while the patient is in the home. This position will typically work daylight Monday thru Friday, rotating holidays, call and every 3rd weekend.
Responsibilities
- Independently provides skilled nursing services in the home and teaches nursing care which will be given by others in his/her absence.
- Possesses the ability to obtain and interpret clinical information considering age specific needs, and communicates findings to patients. Promotes and maintains the health of individuals, families, and the community through teaching, counseling, and appropriate rehabilitative measures. Acts as the physician extender in the patients¿ homes.
- Identifies patients who have physical and/or emotional illnesses through observation and! analysis and helps patient¿s families¿ secure appropriate m! edical, hospital, or other needed care.
- Achieves continuity of patient care by actively participating in patient care conferences. Acts autonomously and provides patient education to address problems of varying complexity from admission to discharge.
- Identifies learning needs and actively pursues required knowledge and skill through in-services, higher education, and other resources.
- Demonstrates effective communication skills and establishes priorities in implementation of nursing care that reflects cost effective practice. Independently completes assignments as scheduled including weekend and on-call rotations with required paperwork submission in a timely and accurate manner.

- Graduate of an accredit school of professional nursing required.
- Bachelors degree preferred.
- UPMC/Jefferson Regional Home Health, LP approved certification preferred.
- One (1) year of nursing experience in an Acute Care Facility or Skilled Care Facility
REHAB PROFESSIONAL REGISTERED NURSE
Current Rehabilitative or Orthopedic Nurse Certification P
Membership in the Pennsylvania Rehabilitation Nurse Society P
INFUSION PROFESSIONAL REGISTERED NURSE
Oncology or Infusion Certification P
One (1) year of acute care IV therapy experience P
One (1) year of obstetr! ical experience or successful completion of obstetrics competency training R
PEDIATRIC PROFESSIONAL REGISTERED NURSE
One (1) year of acute care pediatric experience R
MENTAL HEALTH PROFESSIONAL REGISTERED NURSE
Certification in psychiatric nursing by the American Nurse Association P
Master's Degree in psychiatric or mental health nursing:
Must have nursing experience within the last three years in the psychiatric or mental health unit of a hospital, a partial hospitalization program, an outpatient psychiatric clinic, or psychiatric home care
Bachelor's Degree in nursing:
Must have one year of recent (within the last three years) nursing experience in the psychiatric hospital or mental health unit of a hospital, a partial hospitalization program, an outpatient psychiatric clinic, or psychiatric home care.
Diploma or Associate Degree in nursing:
Must have two years recent (within the last three years)! nursing experience in the psychiatric or mental health unit of a hospital, a partial hospitalization program, an outpatient psychiatric clinic, or psychiatric home care.
Licensure/Certifications
- Basic Life Support OR Cardio Pulmonary Resuscitation
- Registered Nurse
- CPR within 30 days of beginning employment
- Act 33/34/73 clearances required.
