General Summary:
Under general supervision, provide patient information to referring OB/GYN and Pediat! ric offices, assess the need for and provide continuing education to referring hospitals, create networking and outreach opportunities, and market our NICU/MFM departments. Also assist families with general support needs, ensure their continued access to care on an outpatient basis, work with CYS/Home care agencies to ensure outpatient care is continued, and other general duties as needed by WCS.
Duties and Responsibilities:The following are essential job accountabilities:
Serve as a point of contact to all regional OB/GYN and Pediatric offices for patient information and updates, educational opportunities, and marketing.
Coordinate outpatient care and help to ensure patient follow-up.
Notify referring providers when a patient is admitted and provide status updates as needed.
Follow patient progress via continued contact with nursing staff.
Participate in NICU discharge planning rounds on Thursday afternoons.
Maintain two way communications with referring provider! s’ offices in an effort to maintain and improve the process of care coordination.
Report outcomes/statistics to Women and Children Service Line leadership on a monthly basis.
Identify patient and provider needs and further develop methods to meet those needs.
Ensure a smooth transition of inpatient to outpatient care for mothers and babies.
Provide opportunities for community outreach and education as requested.
Protect patient confidentiality by promoting professional staff communications.
