SUMMARY

Housing for Health (HFH) provides housing and supportive services to the Department of Health Services (DHS) clients who are homeless and who have complex medical and/or behavioral health conditions. Since its inception, HFH has expanded to provide a continuum of housing and supportive services to unhoused country residents. HFH is now one of the two largest homeless housing and services entities in Los Angeles County.

In response to the homelessness crisis in Los Angeles (LA), the LA County Departments of Health Services (DHS), Mental Health (DMH), and Public Health (DPH) are coming together to form regional, field-based, multidisciplinary Interim Housing Outreach Program (IHOP) teams to address gaps in behavioral health and physical health services among functionally disabled people experiencing homelessness (PEH) in interim housing (IH) settings. These teams will perform comprehensive assessments of high-risk PEH in IH settings, refer them to supportive services, increase their function and safety within their current environments, and assist them with transitioning them to higher levels of care, as appropriate. These IHOP teams will serve clients in shelters, motels, and other temporary housing locations in all eight Service Areas in Los Angeles County.

The Patient Relations Representative (PRR) for the IHOP program will be responsible for ensuring that IH clients are actively enrolled in Medicaid, empaneled to primary care, and connected to CAL AIM benefits (such as CS and enhanced care management [ECM]) as well as other health plan services (transportation/interpreter services, etc.) The PRR will also assist the IHOP team in making referrals and tracking those referrals to ensure that the client referrals resulted in visits and positive outcomes. The PRR will also assist the IHOP team in other client outreach and engagement activities.

ESSENTIAL JOB FUNCTIONS

  • Interviews patients or their relatives and/or representatives to obtain financial and other eligibility data and assists patients in preparing, completing, and processing Medi-Cal and other healthcare program applications.
  • Collects data and enters patient information into ORCHID,as well as other information systems.
  • Interprets provisions and requirements of the various governmental healthcareprograms to provide a plan that is beneficial to the patient and the department.
  • Analyzes financial and eligibility data and reviews medical records to establish patient eligibility for a variety of federal, State and County healthcareprograms, such as Medi-Cal, CALAIM community supports and ECM programs; completes the necessary documents within time limits specified by the appropriate governmental agency.
  • Determines patients' financial ability to reimburse the County for healthcare services; interviews and corresponds with patients, responsible relativesand/or representatives, attorneys, employers, agencies, and insurance companies to obtain, verify, or clarify information.
  • Determines whether patients have healthcare coverage and assists the client with establishing outpatient providers for routine health care. Assists with setting up transportation services for clients to attend appointments.
  • Identifies problem situations and refers patients to other hospital departments, agencies, or community resources when appropriate.
  • Assists the clinical support team with appointment scheduling and other patient outreach/engagement activities.

EDUCATION AND EXPERIENCE
(TRAINING AND EXPERIENCE)
Two years of experience in the service of the County of Los Angeles as a Patient Resources Worker -OR- Three years of experience in a healthcare setting at the level of Patient Resources Worker with responsibility for performing patient access related work in ED/Urgent Care registration or other walk-in registration, admitting, financial screening and eligibility verification, and scheduling areas.


CERTIFICATES, LICENSES, CLEARANCES

  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
  • Successful clearing through the Live Scan and the Health Clearance process with the County of Los Angeles.

WORK ENVIRONMENT

Work Hours: M-F 8-4:30

General Office Setting, Indoors Temperature Controlled.

Majority of work will be office-based although the IHOP team may require staff to go to an interim housing location if a large volume of clients can more efficiently be assisted on site.
