The Care Coordinator for the Inpatient Units will work collaboratively with the assigned inpatient Manager Patient Care and the department Supervisor in plann! ing, administration, evaluation of the shift operations of the department goals and strategic initiatives. The Care Coordinator assures for the shift assigned that financial, clinical and human resource targets and goals are achieved. The Care Coordinator has shift responsibility for the patient satisfaction, core measures, staffing targets and human resource intervention as appropriate. The Care Coordinator participates in staff orientation and education as delegated by the Supervisor.
Knowledge, Skills, Abilities: The Care Coordinator demonstrates skill in applying and modifying the principles, methods and techniques of professional nursing care to manage patient care. Possesses knowledge of organizational policies, procedures, and standards of care. The Care Coordinator is skillful in identifying shift related problems and aberrations of inpatient progress including the recommendation of appropriate solutions that are communicated clearly and professionally vi! a verbal and written documentation to the Supervisor or Manage! r Patient Care. The Care Coordinator is adept at building and sustaining collaborative working relationships with patients, employees, physicians, various departments and vendors. This individual utilizes concepts of continuous improvement and lean methodologies in the resolution of all patient care and nursing activity issues. The Care Coordinator reacts calmly to all emergency situations. This individual is articulate and skilled in written, verbal and non-verbal communication. Must maintain a professional image and demonstrate strong, principled customer service skills in all aspect of the role. Hours of work require flexibility to all shifts and weekends to meet all job requirements and coverage requirements of other Care Coordinator positions.
Education: Graduate of an accredited school of nursing. Bachelor of Science in Nursing is preferred. Must obtain certification within two years of accepting position. Commitment to professional development through form! al and informal course work in management and clinical specialty.
Experience: Three or more years of acute care nursing experience required.
License/Certification/Registration: Licensed as a Registered Nurse in the Commonwealth of Pennsylvania . Within 90 days of employment, will obtain required certification card for specialty area such as: American Heart Association BLS and ACLS, the American Academy of Pediatrics - PALS or American Heart Association Neonatal Resuscitation Program â€" NALS.
Professional Affiliations: Membership in professional organization highly recommended.
Physical Requirements and Working Conditions: Requires handling and lifting patients, manual and finger dexterity, eye-hand coordination. Requires standing and walking for extensive periods of time. Requires heavy lifting up to 40 pounds by self and up to 100 pounds with assistance. Requires corrected and color vision and hearing to normal range. Requires w! orking under stressful conditions and working irregular hours. May invo! lve exposure to communicable diseases and/or body fluids. "
