Clinical Documentation Reviewer
Required Candidate Background:
Strong knowledge of clinical documentation standards including SOAP or APSO, H&P structure, progress notes, discharge summaries and deciphering billing provider dictations/handwriting. Comprehensive knowledge of anatomy and physiology possibly a clinical background. Understands the ICD-9 Volume I, II and III code choice process preferably medical coding background. Diagnoses and Inpatient Procedure coding preferred to CPT coding.
Job Description:
The candidate will be engaged full time in pediatric clinical documentation review to determine gaps and improve the specificity necessary to meet ICD-10 coding demands ensuring the overall quality, accuracy and completeness of clinical coding. The goal of this project is to determine at a granular level the difference between current documentation specificity and ICD-10 required specificity and review these results with project leaders and division clinical sponsors.
Qualifications:
BS degree in health information management, health sciences or a related field Strong communication skills with clinical audiences are a must Strong knowledge of anatomy and physiology is a must At least 5 years of acute care hospital experience with a strong DRG, MS-DRG, APR-DRG background Clinical documentation improvement experience is heavily favored Working/developing knowledge of ICD-10-CM and PCS is preferred Certification as an RHIA, RHIT preferred | RN certification preferred Experience with Chartmaxx and Epic chart review is a plus CCA or CCS is a plus | CIDP or CCDS is a plus | AHIMA ICD-10 Certified Trainer is a plus
