Children's Hospital of Pittsburgh of UPMC, located in the Lawrenceville area of the city of Pittsburgh, is currently seeking community health RN to work on a full-time basis on a daylight shift (8:30am - 5:00pm), including evenings and weekends as needed, within the FCC Wilkinsburg department. The community health RN will support functions in the Wilkinsburg area. The community health RN functions as a nurse to provide support services to teachers, parents, children at-risk and others in various community settings. He/she provides services such as care coordination, general classroom consultation, modeling of healthcare practices, in-service training, acting as a liaison with primary care physicians, and providing direct care to ch! ildren. He/she also writes and implements individualized devel! opmental healthcare plans.

Responsibilities

  • provide teaching and consultation to teachers and others in group settings regarding general healthcare promotion and prevention techniques (i.e., universal health precautions)
  • provide education on health-related topics as requested by community groups, parents, and other partners. When working in the Family Care Connection (FCC) Department, will provide pre and post natal medical home supervision and referral support
  • identify health needs and stresses prevention and provides guidance on growth and development. In FCC provide appropriate referral and follow-up
  • provide direct teaching and service to children, teachers, and parents in classrooms and in community settings by modeling individualized medical and healthcare procedures. (When working in the FCC, maintains hands-on pediatric skills with a bi-annual rotation through a Primary Care Center)
  • collaborate with an in! terdisciplinary team of professionals and parents to identify individual health needs, and write and implement Individualized Developmental Healthcare
  • plan of goals and strategies for children with medical conditions. (FCC nurse also works on family goal plans)
  • liaison with the child's primary care physician for feedback and approval of the healthcare plan
  • conduct formal and informal (on-the-spot) in-service training and mentoring of teachers and child care staff regarding medical and developmental issues of children at-risk
  • offer healthcare representation on school and agency health advisory groups that develop policies for children with medical needs
  • accept consultation from pediatrician on the primary care team
  • assist in conducting research on healthcare outcomes in early childhood settings
  • assist in writing grants and gaining interagency contract
  • attend annual Hospital and departmental fire/safety a! nd infection control training and maintain current knowledge of safety ! procedures within the Hospital and department
Basic Qualifications

  • 3 years of experience in community health, public health, school, or balanced clinical and non-traditional settings required
  • bachelor's degree in Nursing preferred
  • interpersonal skills required working in a team effort
  • ability to work independently and demonstrated decision making abilities
  • excellent oral and written skills
  • ability to coordinate, prioritizes, organize and meet deadlines
Licensure/Certifications

  • license to practice nursing in the Commonwealth of PA required
  • driver's license required
  • Act 33 Clearance will be required within 30 days of hire
  • Act 73 Clearance will be required within 30 days of hire
